Myth: I Dont Need Statins Or Other Medicines For My Cholesterol I Can Manage My Cholesterol With Diet And Exercise

Michael Flynn charged with making false statements to the FBI

Fact: Although many people can achieve good cholesterol levels by making healthy food choices and getting enough physical activity, some people may also need medicines called statins to lower their cholesterol levels. Guidelinesexternal icon also suggest that other medicines in addition to statins may be needed to help control cholesterol.2

People who may need statins or other medicines to manage cholesterol levels include the following:

  • People with familial hypercholesterolemia or people with very high levels of bad cholesterol. FH is a genetic condition that causes very high LDL cholesterol levels beginning at a young age. If left untreated, cholesterol levels will continue to get worse. This greatly raises the risk for heart disease, heart attack, and stroke at a young age.
  • People with cardiovascular disease . People with CVD may already have narrowed arteries because of too much plaque. Medicines that lower cholesterol may help reduce the risk for heart attack or stroke.
  • People with diabetes.Type 2 diabetes lowers HDL or good cholesterol levels and raises bad cholesterol levels. This combination raises your risk of heart disease and stroke.

Other groups of people may also need medicines to manage their cholesterol, including people who have a high risk for CVD. Always talk to your health care provider about the best ways to manage your cholesterol.

Dietary Fat And Obesity

Epidemiologic Studies

As noted in , epidemiologic studies indicate that average caloric intake has been decreasing while mean body weight and body mass index have increased. There is no epidemiologic evidence indicating that total fat intake per se, independent of total caloric intake, is associated with increased adiposity in the overall population. Although some diseases commonly associated with obesity are associated either with an increase in type of fat in the diet or with the percentage of fat in the diet , no clear association of either percentage or type of fat has been associated with human obesity in epidemiologic studies. Thus, although the type or amount of dietary fat may be associated with diseases common in obese individuals, obesity itself has not been found to be associated with dietary fat in either between- or within-population studies.

Clinical Studies

In one study , caloric compensation was studied in 24 women who consumed in sequence three 2-week diets with low , medium , and high fat contents, similar in appearance and apparent palatability. Subjects on the low-fat diet spontaneously consumed 11.3% fewer calories, and those on the high-fat diet consumed 15.4% more calories than did those on the medium-fat diet. Thus these diets, differing in fat content and consumed spontaneously, resulted in significant short-term changes in body weight due to changes in caloric intake.

Saturated Vs Unsaturated Fats

Different foods contain different amounts of fat with different proportions of saturated and unsaturated fatty acids. Some animal products, like beef and dairy products made with whole or reduced fat milk like yogurt, ice cream, cheese and butter have mostly saturated fatty acids . Other animal products, like pork, poultry, eggs, and seafood have mostly unsaturated fats. Industrialized baked goods may use fats with high unsaturated fat contents as well, especially those containing partially hydrogenated oils, and processed foods that are deep-fried in hydrogenated oil are high in saturated fat content.

Plants and fish oil generally contain a higher proportion of unsaturated acids, although there are exceptions such as coconut oil and palm kernel oil. Foods containing unsaturated fats include avocado, nuts, olive oils, and vegetable oils such as canola.

  • Hong Kong

A 2004 review concluded that “no lower safe limit of specific saturated fatty acid intakes has been identified” and recommended that the influence of varying saturated fatty acid intakes against a background of different individual lifestyles and genetic backgrounds should be the focus in future studies.

The WHO/FAO report also recommended replacing fats so as to reduce the content of myristic and palmitic acids, specifically.

Monounsaturated Vs Polyunsaturated Fat

Assuming given that unsaturated fatty acids are generally healthier than saturated ones , another question that has gained attention in recent decades is the risks and benefits of monounsaturated fatty acids versus polyunsaturated fatty acids .

The most common fatty acids in human diet are unsaturated or mono-unsaturated. Monounsaturated fats are found in animal flesh such as red meat, whole milk products, nuts, and high fat fruits such as olives and avocados. Olive oil is about 75% monounsaturated fat. The high oleic variety sunflower oil contains at least 70% monounsaturated fat.Canola oil and cashews are both about 58% monounsaturated fat.Tallow is about 50% monounsaturated fat. and lard is about 40% monounsaturated fat. Other sources include hazelnut, avocado oil, macadamia nut oil, grapeseed oil, groundnut oil , sesame oil, corn oil, popcorn, whole grainwheat, cereal, oatmeal, almond oil, sunflower oil, hemp oil, and tea-oil Camellia.

Polyunsaturated fatty acids can be found mostly in nuts, seeds, fish, seed oils, and oysters.

Food sources of polyunsaturated fats include:

Food source

Insulin resistance and sensitivity

The large scale KANWU study found that increasing MUFA and decreasing SFA intake could improve insulin sensitivity, but only when the overall fat intake of the diet was low. However, some MUFAs may promote insulin resistance , whereas PUFAs may protect against it.

Myth: All Cholesterol Is Bad For You

Fact: Some types of cholesterol are essential for good health. Your body needs cholesterol to perform important jobs, such as making hormones and building cells. Cholesterol travels through the blood on proteins called lipoproteins. Two types of lipoproteins carry cholesterol throughout the body:

  • LDL , sometimes called bad cholesterol, makes up most of your bodys cholesterol. High levels of LDL cholesterol raise your risk for heart disease and stroke.
  • HDL , or good cholesterol, carries cholesterol back to the liver. The liver then flushes it from the body. High levels of HDL cholesterol can lower your risk for heart disease and stroke.

When your body has too much LDL cholesterol, it can build up in the walls of your blood vessels. This buildup is called plaque. As your blood vessels build up plaque over time, the insides of the vessels narrow. This narrowing can restrict and eventually block blood flow to and from your heart and other organs. When blood flow to the heart is blocked, it can cause angina or a heart attack.

Who Plan To Eliminate Industrially

WHO today released REPLACE, a step-by-step guide for the elimination of industrially-produced trans-fatty acids from the global food supply.

Eliminating trans fats is key to protecting health and saving lives: WHO estimates that every year, trans fat intake leads to more than 500,000 deaths of people from cardiovascular disease.

Industrially-produced trans fats are contained in hardened vegetable fats, such as margarine and ghee, and are often present in snack food, baked foods, and fried foods. Manufacturers often use them as they have a longer shelf life than other fats. But healthier alternatives can be used that would not affect taste or cost of food.

“WHO calls on governments to use the REPLACE action package to eliminate industrially-produced trans-fatty acids from the food supply,”said WHO Director-General, Dr Tedros Adhanom Ghebreyesus. “Implementing the six strategic actions in the REPLACE package will help achieve the elimination of trans fat, and represent a major victory in the global fight against cardiovascular disease.”

REPLACE provides six strategic actions to ensure the prompt, complete, and sustained elimination of industrially-produced trans fats from the food supply:

REview dietary sources of industrially-produced trans fats and the landscape for required policy change.

Promote the replacement of industrially-produced trans fats with healthier fats and oils.

Legislate or enact regulatory actions to eliminate industrially-produced trans fats.

Example Question #: Lipids

Which of the lipids listed below is usually the most abundant phospholipid present in a cell’s membrane?

Possible Answers:

Triglycerides and cholesterol are not phospholipids, so they can be eliminated from the answer choices. Phosphatidylinositol and phosphatidylserine are present in cellular membranes, but they are typically less abundant than phosphatidylcholine, which is the correct answer.

Which of the following is not a correct function of cholesterol?

Possible Answers:

A precursor to steroid hormones, such as vitamin D

A component of animal cell membranes

A precursor to bile salts

Helps maintain rigidity of blood vessels

All of these are normal functions of cholesterol

Correct answer:

Helps maintain rigidity of blood vessels

To answer this question, let’s go through each of the answer choices to see what cholesterol’s functions are.

It turns out that cholesterol is an important component of animal cell membranes. It helps to maintain both the structural integrity of the membrane, as well as its fluidity.

Cholesterol also serves as a precursor for all steroid hormones. In fact, if you look through all the steroid hormones, you’ll find that they all have the characteristic 4-ring structure that cholesterol has.

Dangers Of High Triglyceride Levels

“If triglyceride levels are too high in the blood, they can cause plaque buildup in the arteries,” explained Wright. Scientists are not sure how or why triglycerides cause plaque buildup, but they do know that people with high levels of triglycerides are more likely to have high LDL levels. When plaque builds up in the arteries, artery walls can harden or become thick, which is called atherosclerosis.

Atherosclerosis increases the risk of heart attack, stroke and heart disease. According to the National Institutes of Health , women are especially at risk of heart disease if they have high triglyceride levels. Scientists think that high triglyceride levels can also indicate other conditions that increase the risk of these diseases, such as obesity and metabolic syndrome.

Extremely high triglyceride levels can lead to acute pancreatitis, according to the Mayo Clinic. They may also be a sign of type 2 diabetes, hypothyroidism, kidney disease, and liver disease or rare genetic conditions that deal with metabolism.

Dietary Lipids/fats And Hypertension

Human Studies

Epidemiologic studies suggest that people who subsist on complete vegetarian diets have lower mean blood pressure levels than those consuming omnivorous diets . The specific dietary components involved have not been identified, but they probably do not include sodium since salt intake is similar in many vegetarian and omnivorous groups . The fact that complete vegetarians tend to eat more PUFAs and less total fat, SFAs, and cholesterol has led researchers to design a number of observational, clinical, and community intervention studies to examine the relationship of dietary fatty acids to blood pressure .

A lack of association of hypertension with total fat has also been observed in studies using stroke as a surrogate end point. In a 10-year follow-up study of 7,895 men of Japanese ancestry ages 45 to 68 living in Hawaii with no history of stroke at baseline, Kagan et al. observed an inverse association of total dietary fat with incidence of thromboembolic stroke and total stroke in univariate analysis. This association disappeared when the investigators controlled other factors associated with stroke . No association of total fat with hemorrhagic stroke was seen in either univariate or multivariate analyses.

Fat Digestion And Metabolism

Fats are broken down in the healthy body to release their constituents, glycerol and fatty acids. Glycerol itself can be converted to glucose by the liver and so become a source of energy. Fats and other lipids are broken down in the body by enzymes called lipases produced in the pancreas.

Many cell types can use either glucose or fatty acids as a source of energy for metabolism. In particular, heart and skeletal muscle prefer fatty acids. Despite long-standing assertions to the contrary, fatty acids can also be used as a source of fuel for brain cells through mitochondrial oxidation.

Nutritional And Health Aspects

The most common type of fat, in human diet and most living beings, is a triglyceride, an ester of the triple alcoholglycerolH3H and three fatty acids. The molecule of a triglyceride can be described as resulting from a condensation reaction between each of glycerol’s OH groups and the HO part of the carboxyl group HOC of each fatty acid, forming an ester bridgeOC with elimination of a water molecule H2O.

Other less common types of fats include diglycerides and monoglycerides, where the esterification is limited to two or just one of glycerol’s OH groups. Other alcohols, such as cetyl alcohol , may replace glycerol. In the phospholipids, one of the fatty acids is replaced by phosphoric acid or a monoester thereof. The benefits and risks of various amounts and types of dietary fats have been the object of much study, and are still highly controversial topics.
