Esterified total cholesterol and esterified HDLc were calculated by subtractions of unesterified fractions from total cholesterol and HDLc , . ApoB100 and apoAI were assayed by immunoturbidometric methods . Inter and intraassay coefficients of variance were < 5% for all measurements. All other biochemical and hematological parameters were measured by routine laboratory methods.

Currently, the Friedewald formula is the main method for evaluating low-density lipoprotein cholesterol . Recently, many limitations have emerged regarding its use, including patients with triglyceride levels 400mg/dL, diabetes mellitus, and kidney or hepatic chronic diseases. We analyzed the use of the FF in patients with metabolic syndrome. We selected patients with known metabolic syndrome that fulfilled the National Cholesterol Education Program Expert Panel on Detection, Evaluation and Treatment of High Blood Cholesterol in Adults Final Report and excluded patients with triglyceride levels 400mg/dL and chronic liver and/or kidney disease. Using direct assays, we measured total cholesterol, high-density lipoprotein cholesterol, triglycerides, and LDL-c. Then, LDL-c was estimated using the FF and compared with the LDL-c by direct assay. The sample size was 135 patients. Using the FF, the mean LDL-c value was mg/dL it was mg/dL by direct assay. The correlation coefficient between these two methods was 0.89, with statistical significance . There were no significant differences between the patients with triglyceride levels > 150mg/dL . In conclusion, FF is a good method for estimating LDL-c in patients with metabolic syndrome.
2. Materials and Methods
New Information On Accuracy Of Ldl
Low-density lipoprotein-cholesterol remains of utmost clinical importance it is positioned in clinical trials as a treatment target and is emphasized in worldwide guidelines as the primary cholesterol target. The gold standard of LDL-C measurement has been preparative ultracentrifugation, but given its time requirements and expenses, other methods have been developed as alternatives to estimate LDL-C.
The Friedewald equation was developed in 1972 and estimates LDL-C as: total cholesterol minus high-density lipoprotein-cholesterol minus triglycerides /5, with the latter term serving as an estimate for very low-density lipoprotein-cholesterol .1 Originally developed for research purposes from a sample of just 448 individuals, the Friedewald equation has been widely adopted in clinical practice for several decades.
However, the equation is prone to inaccuracy at low LDL-C and/or high TG levels, where errors in estimating VLDL-C are magnified given its use of a fixed factor of 5 to describe the relationship between TG and VLDL-C. This results in marked underestimation of LDL-C.
In the era when the Friedewald equation was introduced, inaccuracies were tolerated because the VLDL-C estimate was a relatively small proportion of the equation. Lower LDL-C levels were not achievable or strongly recommended as statins and other modern pharmacotherapies were not available. In fact, only 35 individuals in Friedewald’s derivation sample had an LDL-C < 100 mg/dL.1

“Lipid testing is one of the major tools available to assess a patient’s cardiovascular risk, and nearly 200 million Americans have lipid profiles assessed on a yearly basis, with the Friedewald equation remaining the de facto standard in LDL-C estimation,” Sathiyakumar noted.
It is relatively easy and inexpensive to measure total cholesterol, HDL cholesterol, and triglycerides, but not LDL cholesterol or VLDL cholesterol.
In 1972, on the basis of a sample of 448 patients, Friedewald proposed and validated the equation to estimate LDL cholesterol that is used today:
LDL cholesterol = total cholesterol HDL cholesterol triglycerides/5, where triglycerides/5 represents triglycerides/VLDL cholesterol OR
LDL cholesterol = total cholesterol HDL cholesterol triglycerides/2.2.
This equation works very well in patients such as those in the small cohort in which it was derived, with triglycerides less than 400 mg/dL and LDL cholesterol generally greater than 100 mg/dL, but “it starts to break down” in other patients, Martin noted.
To improve the accuracy, in 2013, they developed and validated a modified equation to estimate LDL cholesterol:
LDL cholesterol = total cholesterol HDL cholesterol triglycerides/adjustable factor.
The adjustable factor is based on a patient’s non-HDL cholesterol level and triglyceride level , in a 180-cell table.

This newer LDL cholesterol formula is being adopted by U.S. laboratories as well as others around the world. The best implementation is direct coding the LDL cholesterol estimation in the lab IT system, which automates the process and saves clinicians time. The formula is also available as a mobile device app called the LDL Cholesterol Calculator. It is available on the iTunes App Store and on Google Play, for those whose lab hasnt adopted it.
Researchers hope the new formula will one day be adopted by all labs that process lipid panels, as it can improve patient care.

Low-density lipoprotein-cholesterol remains a key contributor to plaque formation and atherosclerotic cardiovascular disease . Friedewald estimation, however, is prone to inaccuracies at high triglyceride and/or low LDL-C values where inaccuracy in VLDL-C estimation using a “one size fits all approach” constitutes a larger proportion of estimated LDL-C. This subsequently leads to underestimation of calculated LDL-C.2-4 While this imprecision has been historically tolerated, the increased prevalence of high triglyceride states such as diabetes and obesity and novel lipid lowering medications such as PCSK9 inhibitors achieving even lower LDL-C levels provide impetus for improving precision in LDL-C estimation.
Improvements and Updates
However, direct LDL-C assays available commercially use proprietary chemical-based methods, not ultracentrifugation, and are not necessarily reliable. They are not standardized and, in some cases, can be even less accurate than the Friedewald equation.4,5
