Prescription-strength omega-3 fatty acids, available at the US Food and Drug Administration-approved dose of 4 g/day, are a safe and effective treatment for reducing triglycerides among individuals with hypertriglyceridemia, according to a new scientific statement from the American Heart Association .

Before omega-3 fatty acids are prescribed, however, physicians should first address any potential secondary causes of high triglycerides, such as hyperthyroidism or poorly managed type 2 diabetes, as well as implement a strategy to modify diet and lifestyle.

If these changes are not possible or not effective, initiating triglyceride-lowering pharmacotherapy may be required, write Ann Skulas-Ray, PhD , and colleagues in a paper published online August 19, 2019, in Circulation.

The science advisory updates prior AHA documents focused on omega-3 fatty acids, dating back to 2002 and 2011, that predate the advent of purified, prescription-strength formulations. Omega-3 fatty acids exert a dose-related effect on bleeding time however, there are no documented cases of abnormal bleeding as a result of fish oil supplementation, even at high dosages and in combination with other anticoagulant medications.28 High dosages of fish oil may increase LDL cholesterol levels, but the clinical relevance of this finding remains unclear.14,15 Other potential side effects of omega-3 fatty acids include a fishy aftertaste and gastrointestinal disturbances, all of which appear to be dose-dependent.2

Significant amounts of methylmercury, polychlorinated biphenyls, dioxins, and other environmental contaminants may be concentrated in certain species of fish, such as shark, swordfish, king mackerel, and tile-fish .29 In March 2004, the U.S. Food and Drug Administration and the Environmental Protection Agency issued a new statement29 advising women who may become pregnant, women who are pregnant, breastfeeding mothers, and young children to avoid eating some types of fish and to eat fish and shellfish that are lower in mercury. These recommendations29 are summarized in Table 3 . According to a recent survey,30 farmed salmon have significantly higher levels of polychlorinated biphenyls and other organochlorine contaminants than wild salmon. There is disagreement among researchers, however, about the amount of farmed salmon that is safe to eat. High-quality fish oil supplements usually do not contain these contaminants.

How Much Fish Oil Is Too Much

In general, you’re unlikely to have too much fish oil, says Jeffrey Bland, Ph.D., a clinical biochemist and founder of the Institute for Functional Medicine. For most people, the father of functional medicine shares that “there are no serious adverse effects from taking a lot of fish oil,” he says, though some may experience gastrointestinal upset.

But, he adds, people who have personalized bleeding considerations or take blood-thinning medications should talk to their health care provider about dosage before adding a very-high-dose omega-3 supplement to their daily routine. This is because omega-3s have an effect on platelet activity, which can ultimately affect those with specific bleeding health concerns if they are getting exceedingly high doses of omega-3si.e., over 10 grams per day.

To put this “risky” intake level in perspective, however, 10 grams of EPA and DHA would be like taking 10 to 20 times the typical daily dose of a fish oil supplement.

Triglycerides and Disease

Normalization of high triglyceride levels can be difficult. Lifestyle modification with exercise, weight loss, and a diet low in complex carbohydrates can be effective in many individuals. Certain medications, such as the statin drugs and fibrates, can variably lower triglyceride levels. Omega-3 fatty acids in the form of dietary fish or fish oil capsules can help lower triglyceride levels both as a part of a lifestyle program and in combination with pharmacologic therapy.

Potential Side Effects And Risks

Possible side effects of omega-3 supplements and drugs include:

  • Indigestion, heartburn, or burping
  • Diarrhea or flatulence
  • Increased bleeding

Stomach upset is often due to the high fat content of fish oil and can be mitigated by taking fish oil supplements with a meal and early in the day, rather than on an empty stomach or at dinner or bedtime. Another option is to try other omega-3 supplements that do not contain fish oil.

OTC omega-3 supplements that may contain fish should not be used by those with fish allergies. The Rx omega-3s derived from fish are highly purified, yet the risks are still unclear, so those with fish allergies may want to avoid them and opt for plant-based sources instead.

People who take blood pressure medications or anticoagulants, who have hypotension, or who have an increased risk of bleeding or hemorrhagic stroke should seek medical advice before starting omega-3 supplements.

Is It Better Than Other Omega

Fish oil supplements supply EPA and DHA and many also have vitamins A and D.

Meanwhile, general omega-3 supplements may or may not contain EPA and DHA, depending on whether they are derived from fish, sea algae or plant oils.

If your omega-3 supplement is made from sea algae, it has EPA and DHA. Usually, these supplements are high in DHA and low in EPA .

On the other hand, fish oil supplements likely have higher amounts of EPA than DHA, while plant-oil-based supplements give high amounts of ALA.

Although all omega-3s provide benefits, the most beneficial are EPA and DHA .

If you do not eat fatty fish regularly, a fish oil supplement may boost your EPA and DHA levels. However, if you dont consume any fish products, an algae-based supplement is a good alternative.

Otherwise, an omega-3 supplement made from plant oils will help increase your overall intake of omega-3s but likely wont raise your EPA or DHA levels.


Not all omega-3s are equal. While fish oil supplements provide EPA and DHA, most plant sources of omega-3 give ALA which can be difficult to convert into EPA and DHA.

Four grams per day of omega-3 fatty acids, specifically eicosapentaenoic acid and docosahexaenoic acid together or eicosapentaenoic acid alone, is clinically useful as monotherapy or in addition to other therapies to reduce triglycerides after implementing diet and lifestyle changes and addressing any underlying causes, according to an American Heart Association science advisory published in Circulation.

From our review of the evidence from 17 randomized controlled clinical trials on high triglyceride levels, we concluded that treatment with 4 g daily of any of the available prescription choices is effective and can be used safely in conjunction with statin medicines that lower cholesterol,Ann C. Skulas-Ray, PhD, assistant professor in the department of nutritional sciences at The University of Arizona College of Agriculture and Life Sciences in Tucson and member of the writing group, said in a press release.

FDA-approved medications

Despite these benefits with omega-3 fatty acids, patients with very high triglycerides may need multiple mediations in addition to changes in lifestyle and diet to achieve acceptable triglyceride levels, according to the science advisory. Data are limited regarding the use of omega-3 fatty acids with nonstatin therapy for the treatment of these patients.
