The Friedewald equation is used to calculate VLDL-c and LDL-c levels in a lipoprotein estimation test. In this method, total cholesterol, triglycerides and HDL-c are directly estimated using reagents. The formula published by Friedewald WT et al in 19721 was validated by Warnick GR and colleagues in 1990.2

Limitation of Friedewald equation
This method of indirect LDL-c estimation becomes unreliable when the triglyceride levels are high. In that case direct LDL-c measurements have to be resorted to. Friedewald equation was introduced to reduce the time and cost needed for ultracentrifugation in case of direct LDL-c measurement in 1972. Now a days, many laboratories are switching to direct LDL-c measurements in all situations to avoid errors and give better reliability to LDL-c values.
Friedewald equation is not very accurate for estimation of VLDL-c. Friedewald and colleagues had pointed this out in their original publication in 1972 itself. But at that time VLDL-c levels were low compared to LDL-c and the inaccuracy was tolerable.LDL-c values are also not very accurate at the low levels which are currently targeted in those with coronary artery disease. At low levels, Friedewald equation underestimates LDL-c values. This is more important now as hypertriglyceridemia is increasingly common because of obesity, insulin resistance, and diabetes mellitus . 3

Currently, the Friedewald formula is the main method for evaluating low-density lipoprotein cholesterol . Recently, many limitations have emerged regarding its use, including patients with triglyceride levels 400mg/dL, diabetes mellitus, and kidney or hepatic chronic diseases. We analyzed the use of the FF in patients with metabolic syndrome. We selected patients with known metabolic syndrome that fulfilled the National Cholesterol Education Program Expert Panel on Detection, Evaluation and Treatment of High Blood Cholesterol in Adults Final Report and excluded patients with triglyceride levels 400mg/dL and chronic liver and/or kidney disease. Using direct assays, we measured total cholesterol, high-density lipoprotein cholesterol, triglycerides, and LDL-c. Then, LDL-c was estimated using the FF and compared with the LDL-c by direct assay. The sample size was 135 patients. Using the FF, the mean LDL-c value was mg/dL it was mg/dL by direct assay. The correlation coefficient between these two methods was 0.89, with statistical significance . There were no significant differences between the patients with triglyceride levels > 150mg/dL . In conclusion, FF is a good method for estimating LDL-c in patients with metabolic syndrome.

Performance Of Friedewald And Novel Estimates Of Ldl
In considering the implications to patient care, it is important to note that the current standard of practice is not the gold standard LDL-CQ, but rather LDL-CF. To account for this we performed a series of analyses in which the estimated value was considered the reference . LDL-CF was marginally more concordant compared to LDL-CN when LDL-C was > 100 mg/dL. Concordance was significantly higher for LDL-CN compared to LDL-CF when LDL-C was < 100 mg/dL . The greatest difference in concordance between the 2 estimations was observed in patients with LDL-CQ< 70 mg/dL. In this category, LDL-CF correctly classified 2306 patients and LDL-CN correctly classified 2089 patients. However, LDL-CF classified a total of 3239 patients with values < 70 mg/dL whereas LDL-CN classified only 2474 patients in this category, providing relative concordances of 71.2% and 84.4% for LDL-CF and LDL-CN, respectively, if the initial classification were defined by the estimated value .

What Happens If High Cholesterol Goes Untreated
High blood levels of LDL cholesterol and triglycerides have been linked to the development of CVDs, along with other factors such as hypertension and diabetes mellitus. This subsequently leads to underestimation of calculated LDL-C.2-4 While this imprecision has been historically tolerated, the increased prevalence of high triglyceride states such as diabetes and obesity and novel lipid lowering medications such as PCSK9 inhibitors achieving even lower LDL-C levels provide impetus for improving precision in LDL-C estimation.
Improvements and Updates
However, direct LDL-C assays available commercially use proprietary chemical-based methods, not ultracentrifugation, and are not necessarily reliable. They are not standardized and, in some cases, can be even less accurate than the Friedewald equation.4,5
