Fish oil seems to cause an increase in the levels of LDL or bad cholesterol in people with metabolic syndrome. People with metabolic syndrome are those who suffer from a number of conditions such as increased blood pressure, excess abdominal fat, increased blood sugar and cholesterol levels. May Improve Heart Health
Taking a fish oil supplement may help keep your ticker in tip-top shape, according to a body of research.
An August 2016 study in Circulation analyzed the effect that a daily dose of fish oil had on heart attack survivors over a six-month period. Researchers found that this treatment improved the function of the heart, reduced scarring in the undamaged tissue within the heart muscle and decreased biomarkers for inflammation.
Additionally, according to an in the same journal, adults with coronary heart disease who were prescribed a low-dose omega-3 fish oil supplement were 9 percent less likely to be hospitalized or die from the condition.
Whats more, a metaanalysis of 13 randomized controlled trials found that supplementing with seafood-based omega-3s is linked to a lower risk of having a heart attack or developing heart disease, per a September 2019 study in the Journal of the American Heart Association .
Supplementing with marine-based omega-3s was linked to lower triglycerides and larger HDL particles, which are better at removing unhealthy cholesterol and potentially preventing plaque buildup and heart disease, in a February 2020 JAHA study that included more than 26,000 people who identified as female.

Lately, virgin coconut oil has been heavily promoted. Marketers claim that any bad data on coconut oil are due to hydrogenation, and virgin coconut oil is not hydrogenated.
But only a small percentage, 8%, of coconut oil is unsaturated fat, which means only 8% of coconut oil gets hydrogenated. And the yield is mostly stearic acid, the one common long-chain saturated fatty acid that has minimal impact on LDL cholesterol levels. CETP is a plasma glycoprotein secreted by the liver which enables the exchange of cholesteryl esters between HDL and other lipoproteins, such as LDL, thereby promoting hepatic clearance of macrophage-derived cholesterol, via an indirect pathway by hepatic LDLr.
CETP transgenic mice treated with fish oil containing n-3 PUFAs showed significant increases in CETP plasma levels and activity, as well as elevated CETP mRNA expression in the liver . The authors hypothesized a PPAR-related induction of CETP transcription through binding of n-3 PUFAs to PPAR response elements in the CETP gene , since PUFAs have been described as natural ligands for PPAR . In a recent study in hamsters fed a high-fat diet and administered EPA or DHA, distinct results for the CETP-LDL cholesterol pathways were observed . It was found that DHA, in contrast to EPA, led to a significant increase in LDL cholesterol concentrations, an increase in plasma CETP activity as well as elevated CETP mRNA expression in adipose tissue. The effect of EPA on the same parameters, however, was less prominent and suggests that the impact of n-3 PUFAs on CETP and RCT may depend on the type of n-3 PUFA administered . These findings underline the importance of distinct analyses of different PUFA subclasses, as they may exert differential effects on cholesterol metabolism .

Does Fish Oil Raise Cholesterol
Katie had an LDL of 87 mg/dl, HDL of 48 mg/dl, triglycerides of 201 mg/dl. By conventional standards, not too bad.
Reading about the heart health benefits of omega-3 fatty acids, Katie added fish oil. With the preparation she bought, 4000 mg per day provided 1200 mg the omega-3 fatty acids, EPA and DHA. Three months later her LDL was 118 mg/dl, HDL 54 mg/dl, triglycerides 92 mg/dl.
In other words, LDL cholesterol increased by 31 mg. What happened?
Several studies have, indeed, confirmed that fish oil raises LDL cholesterol, usually by 5-10 mg/dl. Occasionally, it may be as much as 20 or more milligrams, as in Katies case, enough for some people to be scared away from continuing this supplement.
Unfortunately, many physicians often assume that its the cholesterol content of fish oil capsules, or some vague, undesirable effect of fish oil. Its nothing of the kind. And, if you were to rely on basic cholesterol values, it does indeed appear to be the case.
But its not.
Another factor: Conventional LDL cholesterol is a calculated value, not measured. The calculation for LDL is thrown off-sometimes considerably-by any reduction in HDL or rise in triglycerides from average values. In Katies case, the rise in HDL from 48 to 54 mg/dl along with the reduction in triglycerides from 201 to 92 mg/dl mean that calculated LDL has become more accurate and rises towards the true measured value. The actual rise in true LDL cholesterol may be small to none.

How To Lower Cholesterol The Bottom Line
So some foods, dietary and even lifestyle changes can make a difference to your cholesterol levels.
All of these simple remedies have been shown to be useful ways to lower cholesterol:-

How Much Is Too Much Omega
Ideal amounts of omega-3s can vary from person to person depending on a number of factors, including age and health concerns. Ask your health care provider about an ideal dosage for your needs. In general, taking more than three grams of fish oil each day may prevent healthy blood clotting and increase the risk of bleeding.
According to the Mayo Clinic, it is not clear if people with fish or shellfish allergies can safely take fish oil. Its always best to talk to your health care provider before taking omega-3 supplements, especially if you have any fish/shellfish allergies.
What about Lovaza? Lovaza is a brand name of omega-3-acid ethyl esters. Common omega 3 acid ethyl esters side effects may include back pain, upset stomach, burping, skin rash, and an unusual or unpleasant taste in your mouth. Very serious side effects can include easy bleeding/bruising, black or tarry stool, or vomit that looks like coffee grounds.
Its also important to note that fish oil can possibly interact with anticoagulant and anti-platelet drugs, herbs and supplements blood pressure drugs, herbs and supplements contraceptive drugs weight loss drugs like orlistat and vitamin E. The daily omega-3 fatty acid requirement for patients with documented CHD or for those requiring lower TG levels is difficult to achieve by fish consumption alone therefore, a fish oil supplement should be considered. Omega-3 fatty acids are available as dietary supplements and as prescription formulations containing either a mixture of both EPA and DHA ethyl esters or as purified EPA ethyl ester.

Improvements In Laboratory Values
A Cochrane review of 23 RCTs involving 1,075 participants with type 2 diabetes mellitus showed that supplementation with fish oil lowered triglyceride levels by 8.1 mg per dL .1 In treated patients, very low-density lipoprotein cholesterol levels decreased by 1.26 mg per dL , and LDL cholesterol levels increased by 1.98 mg per dL . There was no change in high-density lipoprotein cholesterol levels.
An earlier Cochrane review of 17 RCTs sought to determine the effect of omega-3 fatty acid supplementation on rates of mortality and cardiovascular events.2 The meta-analysis of studies that included adults at risk of cardiovascular disease found that omega-3 fatty acid supplementation significantly reduced serum triglyceride levels . Total and high-density lipoprotein cholesterol levels did not change significantly. LDL cholesterol significantly increased . It should be noted that this study examined omega-3 intake from fish and plant sources.
A systematic review of four RCTs concluded that supplementation with two omega-3 fatty acids in doses of 1.7 g or greater reduced triglyceride levels by at least 10% compared with baseline.3 Patients with higher baseline triglyceride levels had the greatest benefit.
Summary Of Past Results
A 2006 paper published in the journal, Atherosclerosis, is the perfect summary of the body of scientific work done to investigate the effects of fish oil on cholesterols.
The authors of the paper did a systematic review of 21 such studies and found that fish oil consumption significantly reduced triglyceride levels, slightly increased HDL cholesterol, and moderately increased LDL cholesterol.
This review also found that high doses of fish oil supplements produced a greater reduction in triglyceride levels than low doses. Also, patients with very high triglyceride levels saw better results from fish oil supplementation.
While it would seem that fish oil did not improve HDL cholesterol nor reduce LDL cholesterol well enough, the authors of this review insisted that fish oil represents one of the effective natural supplements for reducing the risks of cardiovascular diseases.

The scavenger receptor class B type I is a transporter which, in contrast to ABCA1 and ABCG1, enables passive bidirectional flux of cholesterol between cells and HDL particles .
To understand the impact of n-3 PUFAs on SR-BI, it is crucial to mention that this pathway is involved not only in the desorption of peripheral cholesterol to HDL particles, but also in the hepatic clearance of HDL derived cholesterol . Therefore, in peripheral macrophages, the relative contribution of SR-BI mediated cellular cholesterol efflux has been controversially discussed. Some animal studies have suggested a significant efflux function of peripheral SR-BI , whereas other results from cell cultures and animal models are ambiguous . It therefore appears that peripheral SR-BI mediated cholesterol efflux may play a minor role, as compared to clearance via ABCG1 and ABCA1 .
A recent in vitro analysis by Mashurabad et al. showed that EPA leads to a downregulation in SR-BI via PPAR-dependent mechanisms in enterocytes , suggesting that the implications of n-3 PUFAs on the translation of RCT modifying proteins do not only occur in hepatocytes .
In summary, the available literature shows an effect of n-3 PUFAs on SR-BI expression in hepatocytes, which may translate into elevated plasma HDL cholesterol clearance as part of the RCT. No data is available about n-3 PUFA and cholesterol efflux, which is driven through SR-BI from peripheral macrophages.

But heres what has been shown so far from studies:-

- HDL: Omega 3 can raise HDL levels
So fish oil can lower triglycerides, and is good for lowering cholesterol.
