What The Studies Tell Us

High levels of triglycerides is a common condition linked to an increased risk of heart disease and other adverse cardiovascular effects.

Prescription Omega-3 fatty acids can produce significant reductions in triglycerides in people with very high or high triglyceride levels. Prescription formulations whether EPA only or a combination of EPA and DHA have a similar effect on triglyceride levels.

While the effects of these products are similar in terms of triglyceride reduction, they differ in several other parameters. In particular, omega-3 products which contain DHA may increase LDL cholesterol levels in patients with very high triglyceride levels.

Products containing only EPA do not raise LDL cholesterol levels and are typically well-tolerated.

Products containing both DHA and EPA produced a slight increase in HDL cholesterol while the EPA only formulation Vascepa produced a slight decrease.

Empty Stomach Vs Taken With A Meal Omega 3 Fish Oil

The real difference in absorption virtually vanishes if both forms are taken with a meal. The fats present in meals increases the absorption of EE oils.

The difference in absorption also lessens if you take both forms of fish oil over a long period of time. If you only plan on taking fish oil for a week or two, then TG is definitely better. But otherwise, as the President of Tersus Pharmaceuticals recently put it to me, Saying TG fish oil is better is like judging the winner of a marathon twenty minutes into the race.

If you plan on taking fish oil for the long-term, dont sweat the minor difference.

In my opinion, this is not worth the 2X premuim. Why? Because both forms are perfectly safe and the pharmacodynamics are exactly the same.

In other words, both will make you healthier if youre suffering from Omega-3 deficiency. And ultimately, isnt that what we all really want from supplements, to make us healthier?

If Omega-3 must be consumed in the form nature intended, then only eating fish will do. Supplements all kinds fall far short.

Is There An Established Upper Toxicity Limit For Omega

The Food and Nutrition Bard of the Institute of Medicine has not established an Upper Tolerable Limit for omega-3s. However, it is generally not recommended to consume more than 3 grams of omega-3 fatty acids from diet and supplements per day except under the guidance and supervision of a healthcare provider.

Rancidity And Safety Questions

While rTG oils, like PRNs products, address some of the ethyl ester bioavailability and safety concerns, they leave plenty of questions still on the table.

First and foremost, theres the question of rancidity. Biochemists will tell you that the more you work and alter an oil, the more trans fats and rancidity-linked byproducts you risk creating. This is a concern for rTG oils, where the omega-3 molecules have not just been chemically manipulated to create the ethyl ester form, but also further processed.

In addition, rTG oils are most commonly sold in capsules. People have the misconception that putting fish oil into a capsule somehow protects the integrity of the oil. Nothing is further from the truth. During the encapsulation process, the oil is exposed to heat and oxygen, and within a month, the oxidation level of the oil can increase significantly . The concern is that long term consumption of rancid fish oils can be unsafe. Indeed, researchers speculate that oxidized oil may contain carcinogens and cause arteriosclerosis .

What About Otc Fish Oil Supplements

It is important to distinguish the prescription formulations from over the counter fish oil products. The studies we have discussed used a high dose of prescription omega-3 fatty acids.

These days, there is a huge range of fish oil products available online and at health stores. It is important to know how much fatty acid they contain and in what ratio. A product that contains 1,000 mg of fish oils may only contain 300 or 400 mg of omega 3s.

To put that in perspective, you would need to take 14 or 15 capsules a day to get the 4 grams of active ingredients recommended by the studies.

Taking so many capsules is not only unpractical but may also cause adverse gastrointestinal effects.

Although over the counter fish oil products are not regulated in the same way as prescription omega-3s, that does not mean you can not get a quality product. It is also worth noting that the 4 grams a day recommendation relates to triglyceride levels and cardiovascular benefits but fish oil has a wide variety of potential benefits in lower doses.

What Is The Difference Between Ethyl Ester And Triglyceride Fatty Acid Fish Oils

Sports Research Omega

By Dustin Moore August 10, 2017

Triglycerides are the fat form most commonly found in nature, and wild fish. This structure is comprised of three fatty acids + glycerol backbone. When a fish oil is refined or molecularly distilled there is an introduction of alcohol which is used to remove the glycerol backbone of natural triglyceride fatty acids. What you are left with is the ethyl ester form of fatty acids comprised of one fatty acid + an ethanol backbone. This process of molecular distillation or refining is typical when concentrating EPA and DHA omega-3 fatty acids specifically. Pure Alaska Omega Wild Salmon Oil is unrefined and the fatty acids are left in their natural triglyceride state meaning the fat is recognized as easy-to-absorb when consumed.

Are Fish Oil Supplements Safe

Omega-3 fatty acids are generally regarded as safe, however some adverse side effects have been observed in trials using prescription Omega-3s.

Espanova has been linked to adverse gastrointestinal effects including nausea, diarrhea and vomiting. The EPA only medication Vascepa has not been linked to adverse gastrointestinal effects. The most commonly reported side effect in people using Vascepa was joint pain.

Because of the potential adverse side effects, the AHA recommends taking omega-3 supplements with a meal.

Omega 3 Fatty Acids And Triglycerides

Studies have consistently revealed that omega-3 fatty acids can reduce triglycerides. According to the American Heart Association, a dose of 4 grams a day can be used wither as an individual treatment or used along with other treatments for high triglyceride levels.

Note again that the AHA only endorses the use of prescription omega-3 fatty acids.

To date, no head to head studies have been conducted into the comparative effects of DHA and EPA. The majority of studies have looked at the effects of supplements that contain a combination of both acids.

However, in 2015, a review of the comparative benefits of prescription omega-3s was published. This review is highly informative and affords us an opportunity to examine the relative merits of the main types of fatty acid.

Furthermore, the review involved only the analysis of prescription omega-3 fatty acid medications :

  • EPA only : Vascepa
  • Combined EPA and DHA : Lovaza, Epanova and Omtryg

Studies have found that omega-3 fish oil supplements containing EPA and DHA at the recommended dose of 4 g a day can reduce the level of triglycerides by 30% or more in patients with very high triglycerides .

In this group, vascepa the EPA only formulation did not raise LDL cholesterol levels, However, the combination medications containing both DHA and EPA did cause an increase in LDL cholesterol.

In this group, the EPA only formulation medications did not cause a significant increase in LDL cholesterol.

Fish Oil Form It Matters

When we first started developing our fish oil product, our main concern was quality and purity of the fish oils used. From catching the fish, getting it pressed for oil, storing it, and then shipping it halfway across the world to be encapsulated, theres great potential for things to go wrong. Its no secret that theres a huge variation in the quality of oils on the market, and we wanted to ensure ours was on the right side of the bell curve.
In performing our competition analysis to see what other companies were up to, we came across the following copy on Nordic Naturals site, a perceived leader in quality and purity in the fish oil industry.
This piqued our interest as we had also heard from others that the triglyceride form of fish oil was more readily absorbed and the superior form. Nordics website only posted one study, and we took the claims with a grain of salt until we could do our own research. Was there another important characteristic of fish oil besides the quality and purity of the oils? We were going to find out.

Ethyl Esters Vs Triglycerides It May Not Really Matter

When it comes to determining whether the triglyceride form of omega-3 is somehow better or safer than the ethyl ester form, this post provides some food for thought, but at the end of the day it does not take sides. The post suggests that after reviewing the scientific literature in terms of bioavailability, safety or efficacy, that the differences are minor and cannot be considered to be significantly different. It should be noted that Scientific Based Health, where the post appears, sells dietary supplements containing omega-3 fatty acids from both forms.

But back to ethyl ester. There is a third wayin addition to conventional diet and nutritional supplementsto access the benefits of omega-3 fatty acids. And that option is prescription drugs.

There are currently two FDA-approved prescription fish oil medications available, both using ethyl esters. Lovaza combines EPA and DHA , while Vascepa includes EPA only .

This article mentions that a medical food derived from fish oil, Vascazen, is also available. Each 1-g Vascazen capsule contains at least 900 mg of ethyl esters of omega-3 fatty acids sourced from fish oils and includes approximately 680 mg of EPA and approximately 110 mg of DHA.
