Thomas Dayspring is a nationally recognized lipidologist. That means hes an expert on cholesterol, fat, and oil metabolism. And Peter Attia is no slouch in this area. But be warned that video is very technical. Less than 600 people have started even one of these podcasts. We can only guess how many finished the full 5 hours. I did.

Some information wasnt that new. For example, Attia and Dayspring mentioned that the Framingham study indicates that TG/HDL is 5 times more predictive of heart attack risk than LDL.

They also mentioned that lipoprotein particles pass both cholesterol and fatty acids around like hot potatoes. A few of their concepts also seemed to contradict some basics mentioned later in this article.

Why would I focus this article on such simple concepts?

Because the basics of triglycerides and HDL are critical to health. But the complicated version just isnt understandable, let alone actionable.

I did a simple version of the TG/HDL in a video 3 years ago. It was one of our popular videos.

Many people told me the information saved their lives. Chuck Smith is one of those people.

Getting Control Of High Triglycerides

As with many high cholesterol cases, a clinical review published in the November 2019 European Heart Journal found that people with high triglycerides tend to have poor lifestyle habits, such as a high-fat diet, inadequate exercise, and high alcohol consumption.

While medications like statins can help control high triglyceride levels, according to the American Heart Association, healthy lifestyle changes to counter those bad habits and control high triglycerides are even more important than medication.

Here are some of those choices.

Time to get your cholesterol checked. Okay, but which test should you get? Its not so simple anymore. Here is a rundown of some of the choices and their pros and cons:

Total cholesterol. This is the simplest and least expensive test. The test doesnt require any sophisticated lab work, either. The simple, do-it-yourself home cholesterol tests measure total cholesterol.

A lipid screening is a test to look at the levels of the fats in the blood. In the past, doctors felt that children and teens were not at risk for high cholesterol levels. But we now know that children and teens are at risk. This is due to things such as:

  • Being inactive from too much screen time and not enough exercise

  • High-fat or high-sugar diets

  • Family history of high cholesterol levels

Children and teens with high cholesterol are at higher risk for heart disease as adults. Keeping blood cholesterol levels in the normal range reduces this risk.

Whats The Difference Between Hdl Ldl And Vldl

Cholesterol is a lipoprotein made of fat and protein. It is divided into three categories: high-density lipoprotein , low-density lipoprotein , and very low-density lipoprotein , and each impacts your body differently. HDL is considered good cholesterol as it helps move bad cholesterol from other parts of your body to your liver for removal. LDL is considered bad cholesterol because high levels can lead to the buildup of plaque in your arteries, said Chiadika. VLDL is considered bad cholesterol as well but it mainly carries triglycerides, the most common type of fat in your body. Excess levels can also contribute to health complications such as heart disease.

How Can You Naturally Lower Tg And Increase Hdl Levels

The most effective ways to improve your TG/HDL are:

  • Lose weight. Body fat used to be considered as an inert energy storage tissue. Now we know it is an endocrine tissue that releases chemicals that cause insulin resistance, prediabetes, and CV risk .
  • Eat fewer carbs. Get into a lower carbohydrate diet .
  • Exercise. Exercise increases HDL levels and lowers TG levels .
  • Take niacin. It is one of the few supplements that lowers TG , increases HDL cholesterol levels , and lowers LDL cholesterol levels .
  • Get dietary and supplemental omega-3s .

In addition to cholesterol, you might hear about your triglycerides, another kind of fat found in the bloodstream. Women should pay particular attention to this. A high level of triglycerides seems to predict an even greater risk for heart disease in women compared with men, says Michos.

When you take in more calories than you need, your body converts the extra calories into triglycerides, which are then stored in fat cells. Triglycerides are used by the body for energy, but people with excess triglycerides have higher risk of medical problems, including cardiovascular disease. Drinking a lot of alcohol and eating foods containing simple carbohydrates , saturated fats and trans fats contributes to high triglycerides. High levels may also be caused by health conditions such as diabetes, an underactive thyroid, obesity, polycystic ovary syndrome or kidney disease.

Triglycerides also circulate in the bloodstream on particles that may contribute to plaque formation. Many people with high triglycerides have other risk factors for atherosclerosis, including high LDL levels or low HDL levels, or abnormal blood sugar levels. Genetic studies have also shown some association between triglycerides and cardiovascular disease.

Why Is Tg/hdl Important

TG/HDL is important because it is an important predictor of risk for heart disease. It is also a marker of insulin resistance, and insulin resistance is the major cause of prediabetes, metabolic syndrome, and type 2 diabetes.

The Womens Ischemia Syndrome Evaluation study studied 544 women referred for CV disease evaluation . The TG/HDL ranged from 0.3 to 18.4. It was a powerful predictor of all mortality and cardiovascular events.

This chart shows the WISE study range of TG/HDL. Individuals with lower TG/HDL had lower CV disease risk.

High Total Cholesterol Normal Ldl High Hdl Normal Triglycerides

In this case, the high total cholesterol is less troublesome since itâs largely due to elevated HDL . The fact that HDL is high and LDL is normal also makes for a favorable HDL:LDL ratio. Although, extremely high levels of HDL cholesterol can be due to genetics. Generally, HDL levels shouldnât exceed 116 mg/dL for men and 135 mg/dL for women.2

Low Levels May Be Associated With Worse Outcomes In Some Conditions

In one study, people with heart failure and lower levels were more likely to die from heart complications. The levels associated with risk were around 120 mg/dL on average, while levels of 130-149 mg/dL were considered protective .

Upon further analysis, the researchers concluded that only women with heart failure and low triglycerides may be at an increased risk of dying. According to them, low triglyceride levels may point to more advanced stages of heart failure .

In another study, people with lung scarring had triglycerides under 57 mg/dL, which was about 60% lower than the values of healthy controls .

Similarly, people with autoimmune disease had 50-70% reduced triglyceride levels compared to healthy people in a different study. Researchers suggested that low triglyceride levels might actually be a marker of autoimmunity and an overactive immune response .

Its important to note here that low triglycerides in all of these cases are not causing any of these conditions. They are the effect of these conditions. For example, in heart failure, low triglycerides may be due to liver damage due to diminished oxygen supply or increased levels of inflammation.

There is no proof that low triglyceride levels are harmful in and of themselves.

What Are Very High Triglycerides

Just as it sounds, having very high triglycerides means that you have too much of this type of fat in your blood. Your doctor may call this condition severe hypertriglyceridemia.

It is detected through a simple blood test called a lipid panel or lipid profile. This test measures your:

  • Overall cholesterol
  • HDL cholesterol

You will likely be asked to get your blood taken after fastingwhen you havent had any food or drink for eight, and sometimes up to 12 hours. This will show your triglyceride level when your body has cleared any cholesterol from meals youve eaten. A triglyceride level done without fasting can also be very revealing because it often shows that your body is not clearing the cholesterol from food.

Your health care provider will likely use your fasting results to guide treatment, but cases can be very different

Like other types of cholesterol, triglycerides are measured in milligrams per deciliter of blood. There are four categories of triglyceride levels according to the American Heart Association. Someone is diagnosed with very high triglycerides when the amount of triglycerides found in the blood is 500 mg/dL or more under 150 mg/dL is considered normal.

Whats Your Cholesterol Level Anyway

A standard lipid blood test usually measures the concentration of total cholesterol, HDL cholesterol, and triglycerides levels. The LDL-cholesterol level is typically estimated from these numbers using a well-established formula that has been more recently revised and improved by researchers at Johns Hopkins.

So what are your target numbers? According to Michos, an ideal LDL cholesterol level should be less than 70 mg/dl, and a womans HDL cholesterol level ideally should be close to 50 mg/dl. Triglycerides should be less than 150 mg/dl. As Michos notes, total cholesterol levels well below 200 mg/dl are best.

What Health Problems Can High Cholesterol Cause

If you have large deposits of plaque in your arteries, an area of plaque can rupture . This can cause a blood clot to form on the surface of the plaque. If the clot becomes large enough, it can mostly or completely block blood flow in a coronary artery.

If the flow of oxygen-rich blood to your heart muscle is reduced or blocked, it can cause angina or a heart attack.

Plaque also can build up in other arteries in your body, including the arteries that bring oxygen-rich blood to your brain and limbs. This can lead to problems such as carotid artery disease, stroke, and peripheral arterial disease.

Should You Worry About High Triglycerides

Until recently, triglycerides tended to get less attention when looking at cardiovascular risk compared to LDL and HDL cholesterol levels. How Can I Lower My Cholesterol

You can lower your cholesterol through heart-healthy lifestyle changes. They include a heart-healthy eating plan, weight management, and regular physical activity.

If the lifestyle changes alone do not lower your cholesterol enough, you may also need to take medicines. There are several types of cholesterol-lowering drugs available, including statins. If you take medicines to lower your cholesterol, you still should continue with the lifestyle changes.

Some people with familial hypercholesterolemia may receive a treatment called lipoprotein apheresis. This treatment uses a filtering machine to remove LDL cholesterol from the blood. Then the machine returns the rest of the blood back to the person.

How Do I Know If My Cholesterol Is Too High

There are no signs or symptoms when it comes to high cholesterol. The only way to screen for it is through a blood test called a lipoprotein profile which measures your total cholesterol, LDL, HDL, and triglyceride levels. Everyone over the age of 20 should get screened for cholesterol levels every five years. Many people do not know they are affected until a life-threatening event occurs, said Chiadika.

When screening for cholesterol, the numbers below are recommended for an average adult:

  • Total cholesterol level less than 200 milligrams per deciliter is best 240 mg/dL is considered high.
  • LDL cholesterol levels less than 100 mg/dL is best, but this depends on your risk for heart disease.
  • HDL cholesterol levels 60 mg/dL or higher reduces your risk for heart disease.
  • Triglycerides less than 150 is best.

For children, the numbers below are acceptable:

  • Total cholesterol level less than 170 mg/dL over 200 mg/dL is high.
  • LDL cholesterol levels less than 110 mg/dL is best over 130 mg/dL is high.

HDL, LDL, and VLDL are lipoproteins. They are a combination of fat and protein. The lipids need to be attached to the proteins so they can move through the blood. Different types of lipoproteins have different purposes:

  • HDL stands for high-density lipoprotein. It is sometimes called “good” cholesterol because it carries cholesterol from other parts of your body back to your liver. Your liver then removes the cholesterol from your body.

What Causes High Cholesterol

The most common cause of high cholesterol is an unhealthy lifestyle. This can include:

  • Unhealthy eating habits, such as eating lots of bad fats. One type, saturated fat, is found in some meats, dairy products, chocolate, baked goods, and deep-fried and processed foods. Another type, trans fat, is in some fried and processed foods. Eating these fats can raise your LDL cholesterol.
  • Lack of physical activity, with lots of sitting and little exercise. This lowers your HDL cholesterol.
  • Smoking, which lowers HDL cholesterol, especially in women. It also raises your LDL cholesterol.

How Do Triglyceride Levels Compare With Cholesterol Levels

If you have high triglycerides, you are more likely to also have high total cholesterol and high bad cholesterol levels. Many people with high triglycerides also have low good cholesterol levels.

Both triglycerides and cholesterol are measured in mg/dL. However, the guidelines differ for what normal levels are for each of them.
