Many things can cause very high triglyceride levels, such as genetic causes , uncontrolled diabetes, taking certain medicines, being obese or overweight, alcohol use, pregnancy, and thyroid disease. People with high triglycerides should follow a low-fat diet with a goal of weight loss and should not drink alcohol or take certain medicines. People with diabetes are typically instructed to follow a meal plan and take their diabetes medicine as prescribed to improve control of their blood sugar levels.

Some people may need to take medicine in addition to lifestyle changes to lower their triglyceride levels. If you have high triglyceride levels, you should talk to your healthcare provider.

Drug Administration And Pharmacokinetics

The recommended starting dose is 285 mg injected subcutaneously once weekly for 3 months after which the dose should be reduced to 285 mg every 2 weeks. If serum TGs decrease by less than 25% or are not below 2000 mg/dl after 3 months on volanesorsen 285 mg weekly, treatment should be discontinued .

After 6 months of treatment one can consider increasing the dose frequency back to 285 mg weekly if the serum TG response has been inadequate and the platelet counts are in the normal range. Patients should return to 285 mg every 2 weeks if the higher 285 mg once weekly dose does not provide a significant additional TG reduction after 9 months .

While ezetimibe is very effective in blocking intestinal cholesterol absorption it does not interfere with the absorption of triglycerides, fatty acids, bile acids, or fat-soluble vitamins including vitamin D and K.

What Are Normal And High Triglyceride Levels

The National Cholesterol Education Program sets guidelines for triglyceride levels:

  • Normal levels: Less than 150 milligrams per deciliter
  • Borderline high:150 to 199
  • High: 200 to 499
  • Very high: 500 or more

Elevated levels may lead to heart disease, especially in people with low levels of good cholesterol and high levels of bad cholesterol. The same is true if you have type 2 diabetes.

The individuals in this trial were on lipid lowering therapy and the mean baseline LDL-C level was approximately 250-260mg/dL. After 24 weeks of treatment patients in the evinacumab group had a 47% reduction in LDL-C levels vs. a 1.9% increase in the placebo group . This decrease in LDL-C levels was observed after 2 weeks of therapy and was observed regardless of concomitant use of other lipid lowering drugs or apheresis. Notably, in individuals with null-null LDL receptor variants evinacumab resulted in a 43% decrease in LDL-C levels indicating that evinacumab therapy was effective in the absence of functional LDL receptors. As expected, total cholesterol, non-HDL-C cholesterol, and apo B levels were also decreased. Moreover, triglyceride levels decreased 55% and HDL-C levels decreased 30% with evinacumab administration while Lp levels were unchanged.
